LHI Nurse Case Managers work with our clients during all stages of their care, taking the guesswork out of the process. Through regular person-to-person contact, our Case Managers are involved in planning and coordination of health care services, while monitoring progress, to ensure our clients receive high quality, cost-effective care. We currently offer case management to a number of clients, among them, the brave men and women who responded to the World Trade Center disaster in 2001, through the World Trade Center National Responder Health Program. As an organization, we are both proud and humbled to be entrusted with this important responsibility.
Our Nurse Case Managers serve as liaisons between the individual, the health care facilities/providers, the insurance company or other pay sources and any health care resources available. This enables individuals to return to their maximum living, wellness and functional capacity and serves to benefit everyone involved.
