PHA Program

As part of the FEDS_HEAL Program, Logistics Health Incorporated (LHI) is authorized to support the United States Navy Reserve readiness mission by performing Periodic Health Assessments (PHA). The PHA program uses evidence-based protocols for face to face delivery of preventative services, education, counseling, and referrals for treatment.

The PHA program consists of a multi-component process that identifies and records medical readiness related issues, and empowers Service Members to make sound healthy living choices. The PHA program includes the review of the Health Risk Assessment and Physical Activity Risk Factor Questionnaire; Patient Centered Prevention Counseling; Clinical Preventive Services Recommendations; and, a focused exam when a health issue is indicated.

Dental examinations and bite wings (to include dental classification), panographic x-rays, immunizations and/or blood draws may also be ordered with the PHA to help complete the Department of Defense Individual Medical Readiness elements. PHA events are coordinated through the Group Events Department at LHI who manages the event planning from start to finish.
